Brookfield Publishing Co. Lineage Forms

    The Brookfield Publishing Co. Lineage Forms are in two Parts, Part I is to establish descent of a candidate from an immigrant ancestor. Part II is to establish descent of an immigrant ancestor from a qualifying ancestor for a particular Old World Hereditary Society. For candidates establishing descent from an immigrant ancestor with an established lineage, it may not be necessary to submit Part II. When the immigrant is either unknown or doesn't have an established lineage, it will be necessary for a candidate to complete Part II.

    You will be able to access the Lineage Forms in two ways, the first is to click on the pages below and you will be able to view them.

Notes to the New Forms: There have been a number of requests for fill in and print forms and the forms have been included below. Candidates for membership in the Old World Hereditary Societies need to complete the Form in paper form and submit it with the required documentation including an original signature. We are planning to provide for electronic submission for archival purposes, however, with approximately 400 fields we are still working on the process. Once the Part I is working then we will include Part II.

     The Forms indicate that copies of reference materials are to be included with the Forms in addition to the citation of the references. In addition, details of Immigration, including ship, port and other information and authorities for immigration are requested. As the information concerning the immigrant ancestors is included on the Web Site, the information concerning immigration will hopefully be included on the page for the particular ancestor.
